True tarsal cyst

When is a chalazion not a chalazion? When it is a true tarsal cyst.

Have a look at this lump on the eyelid. Then have a look at the everted eyelid. Notes something striking????

IT IS NOT INFLAMED AND THERE IS NO OIL GLAND DISEASE

A chalazion is the clinical manifestation of oil gland disease, something that is SOOOOOO common in the world today.

Calling it blepharitis diverts our attention from the true cause.

So back to the image. This is a TRUE TARSAL CYST, an epithelium lined sac that discharges intermittently through the posterior lid.

The reason this is important is as follows; incising a tarsal cyst like a chalazion leads to recurrence (and a dissatisfied patient), and the correct treatment is formal excision of the cyst, not incision and drainage.

And it’s fun learning how to interpret the clinical findings!
